Rainbow Six Siege's Velvet Shell update brought in a couple new operators as well as many different tweaks, and even more bugs were introduced. Update 2.1.1 should correct many of the known issues, but not everything.
The fixes were scheduled to come out this week, but have since been postponed. They are now slated to be released next week.
Most notably, a couple issues with player weapon skins have been fixed, along with players being stuck in their own drone upon round start.
Triggering Black Mirror with Melee — Mira's Black Mirror Device could be triggered by performing a melee attack from the front.
Players Locked to Certain Camera Angle — Players are sometimes locked to a distant camera angle when loading into a match. There is, however, a workaround: do not press the dashboard button on console or alt-tab on PC during the winning showcase.
Stuck in a Drone After Spawning In — In some instances, a Defender would spawn in while looking through a drone, and will not be able to control their Operator at the round's start.
Charges and Pellets Destroyed Through Black Mirror — Exothermic Charges and Hibana Pellets could be destroyed through the Black Mirror by a melee attack.
Diamond Weapon Skins — Diamond weapon skins were not currently available in the store nor would they show up on weapons in game.
Obsidian Weapon Skin — Obsidian weapon skin was not able to be applied to Year 2 Operators.
Pellets Graphical Glitch — Hibana Pellets had a graphical glitch when used on Coastline.
Headgear Rook and Doc — Rook and Doc shared the same Red Crow headgear.
Rank Charms Clipping — Rank Charms cliped through Mira's hand on the Vector.
Fuze Charge on Windows — On some windows, when a Fuze charge was placed on a barricaded window with unbroken glass, it would be destroyed after the first charge is fired.
Drone Camera Reset — Yokai Drone camera would reset after leaving the drone control view and re-entering it.
Jackal See Light — Jackal couldn't see the red light from active cameras while his Eyenox device is active.
Mira Black Mirror Placement — Mira was sometimes able to place her Black Mirror Gadget on barricaded doorways next to Reinforced Walls.
Canister on Black Mirror — In some instances, the canister that expelled the glass on a Black Mirror could not be interacted with.
DBNO Heavy Breathing — "Heavy breathing" from DBNO state persisted after being revived.
Headgear Textures — Some headgear textures received fixes.

Hit Registry — This remains Ubisoft's top priority and they've tracked plenty of data throughout Season 3. They've come to the conclusion that only less than 1% of all shots are actually unregistered. However, they plan to make it so players feel as though the registry is fixed.
Connection Issues with IPv6 — For now, you should be enabling IPv4 protocol support on your router. Check your router's manual or go to your internet service provider's website to find the information.
Patch 2.1.1 for Rainbow Six Siege brings all of these bug fixes to the live servers and should be out sometime next week.
